World News and Views :: Top of the News U.S. Authorities Investigate Fatal DuPont Accident3:00 PM MST | January 25, 2010 | Kara Sissell The U.S. Chemical Safety and Hazard Investigation Board (CSB; Washington) has launched an investigation into an accident at DuPont’s Belle, WV complex that killed one. The fatality occurred when the worker was exposed to an accidental phosgene release, CSB says. DuPont officials told the CSB that a braided steel hose connected to a one-ton capacity phosgene tank suddenly ruptured, releasing phosgene into the air.
