CSB Details Approach to BP Deepwater Horizon Investigation

CSB says its separate investigation of BP’s Deepwater Horizon accident will examine BP’s recently released internal investigation report “without prejudice.” CSB also says that its own investigation into 2005 BP’s Texas City accident ultimately detailed root causes that were different from those identified in BP’s own Texas City investigation, with CSB’s findings more focused on factors including corporate safety culture, cost cutting, and antiquated equipment....
